Visual Connections is seeking a highly skilled, credentialed Mid-Level Medical Coder (coder) to support coding accuracy, compliance, and audit readiness across federal healthcare programs. This is a non-entry-level role intended for coders who can operate independently in a production or review environment. The coder(s) will be hired as an employee for up to 40 hours per week (flexible scheduling). This is a remote position.

The coder will be responsible for accurate code assignment, documentation review, and compliance validation in alignment with CMS and federal guidelines. This role requires strong clinical interpretation skills, attention to detail, and the ability to apply complex coding standards with consistency.

KEY RESPONSIBILITIES

• Perform accurate assignment and validation of ICD-9-CM and ICD-10-CM

• Review clinical documentation to ensure completeness, specificity, and compliance

• Apply CMS and other regulatory guidance

• Identify and resolve coding discrepancies and documentation gaps

• Support coding audits, quality assurance, and compliance initiatives

• Maintain high accuracy standards (95%+ required) and meet production expectations

• Collaborate with internal teams to ensure alignment with federal and contractual requirements

REQUIRED QUALIFICATIONS

• All coders must have experience in abstracting ICD-9/ICD-10 (blended HCC model) coding and have one or more of the following certifications: Registered Health Information Administrator (RHIA), or Registered Health Information Technician (RHIT), CCS (Certified Coding Specialist), Certified Risk Adjustment Coder (CRC), or Certified Professional Coder (CPC). CPC-A not accepted.

• Minimum 3–5 years of hands-on medical coding experience in a production or audit environment, including abstraction and ICD-9/10 coding of general acute care hospital records (inpatient and outpatient) and physician medical records. Must demonstrate proficiency in applying ICD-9/10 Official Coding Guidelines and relevant AHA Coding Clinic guidance for both inpatient and outpatient settings.

• Knowledge in anatomy and physiology, pathology of disease and medical terminology required.

• Strong proficiency in ICD-10-CM

• Demonstrated ability to interpret clinical documentation and apply coding guidelines independently

• Passing score on a VC/NextStep administered coder assessment must be achieved before further consideration

• Preferred experience working within CMS-RADV, MAO plans, or other government-regulated environments

• Experience, risk adjustment (HCC), or audit/review functions
